1. create an empty form, with slash command

2. Add the form’s fields

3. (optional) Link the form with your google spreadsheet

4. (optional) attach a reward role when submit

5. (optional) send a button linked to that form

Test it out

Show the form (By 2 methods)
- Method 1: Use /form
- Method 2: Use a button linked to the form
Fill your data and press submit
Check your roles, the reward role should be granted (in this case Creative Mind)
